Phlebotom3 Innovations L.L.C. provides mobile healthcare-related specimen collection, occupational testing, compliance, and screening services. Our services are designed to provide convenient, professional collection and testing support for individuals, employers, healthcare organizations, industrial facilities, and other businesses.
Our services may include mobile blood specimen collection, employer drug testing, DOT and non-DOT specimen collections, alcohol testing, DNA specimen collection, insurance examination services, wellness and laboratory specimen collection, OSHA respirator fit testing, and QuantiFERON-TB Gold Plus blood specimen collection.
Medical & Laboratory Disclaimer
Phlebotom3 Innovations L.L.C. provides specimen collection and related services and does not independently diagnose, treat, or prescribe for medical conditions. When laboratory testing is performed, specimens may be submitted to an appropriate reference laboratory for analysis. Laboratory results are generated and reported by the performing laboratory and should be reviewed with the appropriate healthcare provider when medical interpretation or follow-up is necessary.
Occupational & Compliance Testing
Drug, alcohol, DOT, non-DOT, and occupational testing services are performed according to the applicable testing requirements, employer policies, and/or regulatory standards governing the requested service. DOT-regulated testing is performed in accordance with applicable U.S. Department of Transportation requirements.
QuantiFERON-TB Gold Plus
QuantiFERON-TB Gold Plus is a blood test used to help identify TB infection. Phlebotom3 Innovations L.L.C. provides specimen collection and does not diagnose or treat tuberculosis. A test result alone does not determine whether an individual has active TB disease. Additional medical evaluation may be required based on symptoms, medical history, risk factors, and laboratory results.
OSHA Respirator Fit Testing
Respirator fit testing is performed to evaluate whether a specific respirator properly fits the individual being tested. Fit testing does not replace an employer's respiratory protection program, required medical evaluation, training, or other obligations under applicable OSHA requirements.
DNA & Insurance Services
DNA collections are performed for specimen collection purposes and may be submitted to an appropriate laboratory or testing facility. Phlebotom3 Innovations L.L.C. does not independently determine legal, medical, or genetic conclusions from DNA testing.
Insurance examination services are performed according to the requirements of the requesting insurance company, organization, or authorized party. Phlebotom3 Innovations L.L.C. does not determine insurance eligibility, coverage, underwriting decisions, or policy approval.
Client Responsibility
Clients are responsible for providing accurate information and any required orders, authorizations, identification, consent, or documentation necessary for the requested service. Certain services may require a physician's order, employer authorization, laboratory requisition, or other documentation before services can be performed.
No Medical Emergency Services
Phlebotom3 Innovations L.L.C. is not an emergency medical service. If you are experiencing a medical emergency, call 911 or seek immediate medical attention.
By scheduling or receiving services, clients acknowledge that Phlebotom3 Innovations L.L.C. provides collection, screening, testing, and occupational/compliance support services within the scope of the specific service requested and does not replace a physician, laboratory, healthcare provider, employer, or regulatory agency.
Services are subject to applicable federal, state, local, laboratory, employer, and regulatory requirements.
